When Is a Vitamin D Injection Needed Instead of Oral Supplements?
Oral Vitamin D3 (cholecalciferol) sachets and capsules are effective for mild-to-moderate deficiency. An injection is preferred when: serum 25(OH)D is below 12 ng/mL (severe deficiency); the patient has malabsorption syndrome (Crohn's, coeliac, post-bariatric); compliance with daily oral supplementation is poor; rapid repletion is needed (bone pain, muscle weakness, fracture risk); or the patient is elderly, homebound, and cannot reliably take daily supplements. A single IM injection of 300,000–600,000 IU Cholecalciferol (Arachitol or equivalent) can be given once, or divided into two doses 4 weeks apart.
What to Expect from the Vitamin D Injection Visit
The injection is given intramuscularly in the deltoid (upper arm) or gluteal muscle. The nurse brings the Arachitol or prescribed Cholecalciferol vial, a fresh syringe and needle, and alcohol swabs. The injection takes under 2 minutes. Mild soreness at the injection site for 24–48 hours is normal. Your doctor will typically recommend a serum 25(OH)D test 3 months after the injection to assess response and determine if a maintenance dose is needed.
Combining Vitamin D with Calcium Supplementation
Vitamin D alone is insufficient for bone health — calcium intake must also be adequate. Most doctors prescribe calcium carbonate or calcium citrate supplements alongside the Vitamin D injection. immidit nurses will check whether your prescription includes calcium and remind you to take it as directed.
